When completing your EIPP Upgrade Enrollment Form you will need to have handy the following information:

- Teachers Life Member ID number
- Your EIPP Policy number
- Current Coverage details

All of this information is available on the Annual Statement of Coverage right here through our NEW, secure, password-based, MEMBERS feature. By signing up in MEMBERS you are also able to update your profile, download forms, as well as check out your policy details... even update your address.

If you have not already a signed up in MEMBERS it is easy!
You will need to have your Teachers Life member ID number and a policy number handy. Both pieces of information can be found on your EIPP policy certificate.
